Registration Form
You may register by e-mail in the following way: Click here to download the Registration Form in ".rtf " format (rich text format). The file "reg.rtf" may then be opened with virtually any word processor or text editor on any computer platform.
The form may then be filled out by overwriting some of the "periods" with your text.
Then e-mail the completed form (by copying and pasting it into the body of the e-mail AND sending the file as an attachment) together with credit card information requested on the form (if applicable) to the conference coordinator, amy@usc.edu and use as "Subject", ICDEA2004.
Wire Transfers of Fees: Contact Amy Yung amy@usc.edu and give your Name and the Amount you will be transferring. She will send you routing instructions.

You may register by snail-mail in the following way: Download the Registration Form.
Click here for".rtf " format, OR Click here for".pdf " format.
Then print the form and send the completed form to the address given on the form together with a check payable to "USC" denominated in $US (United States Dollars). Allow 10 days for the check to clear the bank.
Cashier's checks drawn outside the United States on Bank of America in $US are acceptable.
