If you wish to print a copy of this application, please follow these instructions:

2. Increase the size of your window to the black line at the top of the application
3. Set your font to Times and the font size to 10 points
4. Print the application

Important Note: If the ICA application does not fit on one printed page, it will not be accepted at ICA Headquarters. In the case that your printer prints the application on more than one page, please call the ICA headquarters at 512-454-8299.
